The role of the caregiver/ILS worker is to assist clients with increasing their independence and self-sufficiency within their community.
- Make your own schedule, independent work environment. No experience necessary. Lots of promotional opportunities.
- Must be able to pass a background study and have a driver's license and an insured vehicle.
Examples of work performed with clients include but is not limited to:
- Finding and accessing community resources (food shelves, housing, employment, etc.), budgeting, and financial management (bill pay, balancing accounts).
- Household management (grocery shopping).
- Meal planning/preparation, sales/coupon use, etc.).
- Community integration/socialization, symptom management, etc.
- It is preferred that caregivers have at least 1 year of experience working with individuals with disabilities and/or mental health diagnoses.
Job Title: Designated Coordinator/Manager (245D)
Salary/Pay: $20-$25/hour DOQ
Schedule: Monday-Friday, flexible during operating hours
Hours: Part-time, 15-20 hours/week
Deadline: Open until filled
The position is responsible for the overall programmatic implementation of the person-centered approach. AM HOMES serves seniors age 65+ and adults age 18+ who have a verifiable disability.
This position has duties that include, but is not limited to:
- Assist and/or complete intakes with person-centered served plan with team members.
- Developing and/or writing and implementation of each client’s person-centered support plans, (CSSP) Coordinated Service and Support Plan, (SMA) Self-Management Assessment, and (IAPP) Individual Abuse Prevention Plan.
- Maintaining all signatures as required by 245-D licensing rules and regulations.
- Maintaining, updating all files as required by 245-D licensing rules and regulations.
- Compliance with 245-D licensing rules and regulations.
- Writing or quarterly and annual reports as needed or directed by the person served and/or team to fulfill person-centered plan.
- Able to problem solve as needed and/or be creative to problem solve.
- Coordination and ensuring the protection from abuse and general well-being of clients.
- Staff orientation, training, and supervision regarded to person-centered needs and services as written in their specific plan.
- The designated coordinator monitors target symptoms, attends psychiatric appointments, and communicates with all team members.
- Be available to receive calls during and outside of work hours from staff with questions or during emergencies.
- Requirements:Ability to pass a background study conducted by the Department of Human Services (DHS);
- Qualify as a Designated Coordinator under 245D licensing rules;
- Valid driver’s license;
- Dependable vehicle;
- Clean driving record;
- At least 2 years of experience working with individuals who have intellectual disabilities and/or serious and/or persistent mental health concerns.
- At least one year of supervisory experience (preferred);
- Experience: StudentsMSW, LGSW, MSW
- Working with persons with Mental Health: 2 years (Preferred)
- Working with persons with Developmental Disabilities: 1 year (Preferred)
- Supervisory: 1 year (Preferred)
Requirements:
